Amazon refunds buyer automatically with no return created
So what's the deal with Amazon automatically issuing refunds when the buyer contacts them and not creating return requests?
Worse, when you contact support they lie about you doing the refund and won't create a return. I've sent them screenshots of the emails that clearly state Amazon refunded the buyer after they contacted them, but will insist their reports say I refunded the buyer. Amusingly they refuse to show these results yet won't acknowledge the emails I get that clearly show Amazon refunded the buyer without my consent.
I've been seeing this happen more and more with orders where the shipping was bought through Amazon and they will outright deny a request. So not only do the buyers get a free item, but you pay Amazon a fee on top of the loss to let them keep it.

Tatiana from Amazon here. I think what you might be seeing here is our prepaid returns policy at work.
Since sellers are required to comply with the general Amazon return policy, most seller-fulfilled returns that are within policy are auto-approved and a prepaid shipping label is provided to the buyer. The funding of the refund will be made by the seller and you'll see this funding debited from your account on file.
However, I am happy to review a recent order and seller support case ID for you if you believe there might be a different issue.
