Product’s serial number, model or type identification
Should state that you take full responsibility for the declaration. Should show the relevant legislation with which the product complies, as well as any harmonised standards or other means used to prove compliance.
Name, signature and position of person signing
The date on which the declaration was issued

contact whoever you buy the product from - they should have the documents.

Jellycat don’t allow sales on amazon, its in their terms and conditions
