Royal Mail Refused At Export - What's Going On?

I send quite a few international orders, last week i had one returned 'refused at export' saying i didnt fill out the CN22 form, i thought ok maby i made a mistake on this one, today i've had another returned 'refused at export' ....
I've just checked on my OBA account and it shows on the order returned today that i put in the correct customs / hs code, i only sell t-shirts so my code is 61099090 , i also fill in the other fields such as the weight, item name / descrption etc.
So why would i get 2 refused at export in the space of 10 days, i've not had any refused at export in over 2 years, well since it initially came about and i wasn't filling it out at first i got a tonne refused, since then ive never had any refused.
My Questions Are:
1. Has something changed that i'm not aware of?
2. How can i check on my OBA / Click & Drop account if i actually filled out the CN22 correctly, yes it shows i've put the correct customs code, but how do i check the other details such as name etc , i'm 98% certain that if the code is there then i've filled the other part of the form correctly, because it does not let you create the label if you don't enter the product name etc
3. If this is an error on Royal Mails part what can i do? International labels are not cheap so returning 2 to me in the space of a week means im £20 out of pocket, if it's my mistake that's fair enough but i really don't think it is, so i'm trying to get to the bottom of it.

I don't think this is the reason for the issue, but are you sure you have that Customs Code right?
When we send t-shirts, it's 61091000(10), I've not used 61099090 before.
Did you print the CN22 and attach it to the parcel?

Same problem here..
We’ve had approximately 18 items returned to us in the past 2 – 3 weeks. 15 of the 18 returned items are from Republic of Ireland.
Spoke to Royal Mail and been advised that even though we “filled in the form correctly” we need to attach the completed cn22 form to each item. Been advised we cannot re-use the old labels
Had to go into settings to change “label format” to include cn22 forms..
“Choose additional options”
then tick
“Generate customs declarations with orders”
Been advised we cannot re-use the old labels so I am currently manually re-doing the rejected items.

Did you print and stick the customs form to the parcel? Because if you didn't, that's your problem.
I've had issues on and off with ROI customs and also with Brazil - Brazil has to do with AIRN not going through to click and drop since an update in December.
ROI seems to on and off have issues with all sorts of things including country of origin and the materials things are made from.
Are your T-shirts definitely made in the UK or are you putting "country of origin" as UK because you're shipping from there?
Best thing to do, if you've printed and attached the form to the parcel that was returned, is to contact Royal Mail's international team through the form at their website /forms/international-service-form (copy that bit onto the end of the web address as the forum doesn't allow external links) and ask them for confirmation on the current situation.

The CN22 label should have IOSS in a circle AND the electronic hand zap symbol indicating that all export data has been completed correctly.
