it’s easy enough to provide one of these. I believe Amazon have a template in their Help section. Have a look.

it’s me of those odd things amazon ask for. It’s fairly simple to put together, as odd as it is to give yourself authorisation. There’s a template for amazon pay below that can be adjusted.
