Baby Product approval

Hello guys,
Please help! I’ve recieved a message saying I require approval to sell my baby product. Amazon is asking if I’m a seller/distributor or a manufacturer, or both. Would I select both if I’m doing FBA PL with a custom made product from china? Amazon is also asking for me to submit pictures of the below:

Clearly display all sides of the product or its packaging

Include model number, product name, or both

Include name and physical location of the business or manufacturer

** Important information must be in the country’s local language

So, what is the model number? Is name and location of business my sole trader details so my full name and home address? Also, would I just submit a word document answering these rather than an image.
