This is confusing-you want the buyer's information? You can't ask them for any of that.

This is common practice by many FBA sellers to include a card or something in the product packaging with contact information.

If your product was for sale at Target or Walmart what would be on or in the packaging? An address, a phone number, a website - all are perfectly acceptable and commonly included on retail packaging.
Amazon can't tell Nike not to include customer service contact information when they sell a pair of shoes. It's when sellers included material explicitly trying to get reviews or drive future sales off Amazon that you're going to get in trouble.

Why? No to phone number for several reasons. Address and email are already available. Customers that contact you need to go through Amazon so a record of all communication is in writing. The least contact we have with buyers is best for us.
