We have a range of cabinets and drawers - that get classified under "clothing storage unit"
We are not able to understand what amazon wants - when it says it wants an image of "Name and address of the manufacturer, importer, distributer, or authorized representative"
We have given it repeatedly and gets rejected - is there a specific format for it? Any help would be greatly appreciated.

From this information, I believe you are working on a Compliance requirement issue. Please review the help page for: "Clothing storage units"
I am pretty sure you have already reviewed this information. If you could provide an ASIN, performance notification or case ID for an item that has been rejected, I could look deeper into this issue for you.
Some additional information I might give you for Clothing Storage Units would be the different safety standards you need to prove compliance with:

- 16 CFR Part 1303 (Lead in Paint and Surface Coatings Testing)
