Order fulfilled by FBA, customer claim not receive

We have recently received a return request claiming that the order was not received. This order was fulfilled by FBA, and the UPS tracking shows that the order was delivered with a signature. Amazon has already issued a refund and deducted the money from our account. Has anyone else had a similar experience? And do you know where and how we can file the claim? Thank you.!
