when you ship a label, you can decide what items are included in the package. Leave that one out. Once you shipped the other 5, that one should remain as "unshipped" (This is unless things changed recently, can never be sure with Amazon. But you should see what I mean as you go through the steps.

I am not sure the advice from Nuts Again will work. It will work if you are simply shipping that one separately -- but not for refunding. You need to confirm as if you were shipping all, then just refund for the one you cannot ship. Wait several hours before doing that.
Naturally you will want to send a note of explanation BEFORE you purchase the shipping label as you probably cannot communicate later.
If there is any shipping charge connected to the book you are not sending, be sure to refund that as well.
