Refurbished/Grade Items – What if Amazon Sends a Faulty One?

Quick question — if we opt for the “grade & resell”, “repackaged & refurbished” options and Amazon ends up sending a faulty unit to the customer, what happens then? 😬
Like, if the buyer leaves negative feedback because of that, who’s actually responsible?
Also, is there any way to avoid this mess in the first place — and if it does happen, what’s the best way to fix it?
