Submit Tax Information Issue

I set up account as individual, using my SSN. Everything went fine until the "Preview and Sign" page; I filled out everything correctly and checked the boxes, but it did not move to the confirmation page after clicking the "Submit Form" button. It stayed on the same page after multiple tries. Not sure what happened.

Can anyone give me some tips?
