Help...I can't reactivate my account

Hi Everyone,

Does anybody knows how can I reactivate my account?
When I try to appeal and submit the documents amazon required, I will automatically receive the response as below.

So, do you have a banner on your Account Health page, as it says ?
Do you have the email you received with the deactivation notice - maybe post it here.
What did that ask you for ?
