Did Amazon know you had an employee with access to your account? If not then that is likely the reason for the rejection.

What your employee did was a big no-no. I would have thought that you would need to write an appeal using Amazon's required 3 point format.

Unfortunately what happens to your account is your responsibility, it doesn't matter who has done the violation on your account but if they have been authorised at some point then whatever happens lays at your door.
I am not sure you should really be showing Amazon a termination letter for your employee as that really is just a matter for you.
Amazon require a plan of action. You have breached a policy and they have given you a policy violation which you can either serve or appeal. Some violations require an appeal to prevent deactivation or they just remain on your account until the violation becomes spent.
My advice would be to accept your mistake and Amazon are after the following from you.
They want assurance first and foremost that if you are able to continue your account, you will not breach this policy again and you have taken reasonable steps to ensure it will not happen again. Just sacking the employee and saying they have gone doesn't explain to them why they were trusted in the first place and what's to stop the replacement employee from doing the same thing?
You need to construct an appeal (preferably not in CAPS) explaining that you have received a policy violation for a breach of policy and then go on to explain why this was breached even if you just accept this was a misunderstanding of the policy or adequate training had not been given to a trusted person on the account resulting in the violation. Apologise and explain that in future and now having a clear understanding that training has been given to ensure that the policy will not be breached in future as you have put steps in to prevent a future breach of policy.
Thank them for their understanding and ask them if there is any other infromation that they would like to further help with an assurance.
I don't know what stage you are at but you are generally allowed to continue an account with violations depending on your track record, other breaches etc but these things will tend to linger but not cause you problems unless you make another slip up.
