There are lots of templates if you google this. You will need to produce one yourself and then upload it in the order page.

If you are VAT exempt you can get Amazon to provide invoices for you automatically. I'm not sure if the below link will work as I'm not eligible for this. If it doesn't in seller central search for Invoice Defect Rate, in the FAQ option under number 9 there is a declaring VAT exemption link. Unfortunately I don't think this will work on orders already received.

Are you Vat registered ? - if not then you cannot give a VAT invoice, you need to issue a receipt, with no mention of VAT.

You do not need to give an invoice/receipt for all orders, only for those marked as fro Business Customers or if requested.

Is this for sales to Business customers ? - if so you can declare your VAT exempt status if you are not VAT registered and Amazon will automatically upload for you. They will not do it for orders already made, so as said above, you will need to produce an invoice/receipt and upload that for any business customers.
