Replacement order loophole scam

Today and lately I have had customer's being refunded through a loophole with replacement orders as of late. Exact scenario has happened 2 times this past month.

Customer orders a product.

A few days later, customer requests a replacement order.

Customer then returns the replacement order and files a a-to-z claim for the replacement order, but still has the original order in their possession.

Amazon grants the claim and refunds the replacement order, since "I didn't refund it." (They didn't pay anything for the replacement order, so I wouldn't refund it)

So customer gets their money back and keeps the (original) order.

I tried appealing the a-z claim and was denied. Tried contacting support, but they just forwarded the case to the same team and was denied.

What possible solutions are there for this?
